Integrating Nutrition into Primary Care
Purpose: Supporting primary care providers and their care teams in 1) preventing chronic disease through evidence-based nutrition strategies and 2) developing a deeper understanding of the social and environmental drivers of health related to food and nutrition. This program will enhance providers’ ability to deliver high quality, patient-centered care. The project described is supported by grant number 6 U3IRH43509‐04‐02 from the Office for the Advancement of Telehealth, Health Resources and Services Administration, DHHS
Audience: We will select health professionals from Oregon who work in primary care settings to participate in this program, including physicians, nurse practitioners, physician assistants, behavioral health clinicians, pharmacists, traditional health workers, administrative staff, and others. Participants are encouraged to participate together as primary care teams. Participants will be chosen based on their interest in the program, geographic distribution, and ability to commit to attending the sessions.
